Spain Jet Crash: 'Wing Flaps Failed'

The investigation into the Madrid Plane Crash, which killed 154 people, has revealed that flaps on the jet's wings failed to open on take off, according to an article on the BBC.

The pilot was unaware, as a warning alarm in the cockpit failed to go off.

According to an article in El Pais, a Spanish newspaper, the failure of these flaps also caused a plane crash in Detroit in 1987.

Since the Detroit accident, El Pais reports, Boeing has advised airlines to check that the flaps work before each flight. However, Spanair only asks that its crews make the checks before the first flight of the day or when both pilots change.
